Theatre at St. Michael’s (TASM) Camp Policies & Forms - 2025

SUMMER 2025
What forms do I need to fill out before camp?
TASM camp forms will include the Camper Information Form and the Camper Medical Form (covering medical, dismissal & release details), as well as (optional) Lunch and T-shirt Forms.
ALL CAMPERS should please fill out the Camper Medical Form and Camper Information Form before their first day of camp. These are part of the registration process, and can be accessed through the registration link below.
2025 tasm Registration login ALL CAMPERS should also:
- share an Emergency Action Plan if they have emergency medications such as Epi-Pen or inhaler
- use this Medication Note for any medications (including OTC) that need to be taken during the camp day
CAMPERS in the Original Musical Session or Shakespeare Production of As You Like It can fill out the Lunch and Shirt forms if they would like to order lunches or shirts. The Techie shirt is available for campers in either session.
