Theatre at St. Michael’s (TASM) Camp Policies & Forms - 2026

We are looking forward to having you with us this summer! All forms can be filled out through the TASM registration Parent Portal. Please don’t hesitate to email Christina Johnston if you have further questions. Thank you!

What forms do I need to fill out before camp?

ALL CAMPERS must fill out the Camper Medical Form and Camper Information Form (covering medical, dismissal & release details) before their first day of camp. These are part of the registration process, and can be accessed through the registration link below. In addition, registered campers will be provided with (optional) Lunch and T-shirt Forms as the summer approaches.

2026 TASM Registration login 

ALL CAMPERS should also:

  • share an Emergency Action Plan if they have emergency medications such as Epi-Pen or inhaler
  • use this Medication Note for any medications (including OTC) that need to be taken during the camp day
